Request forms: These are fillable PDF forms (UPDATED Feb 19, 2010) which can be downloaded, saved to your computer, completed, printed or emailed to MVI as needed. Adobe Reader is required to fill out these forms. If you have any problems completing or saving these forms, you can download the newest version of Adobe Reader by clicking the icon above.
X-ray Interpretation requests:
1. Complete x-ray interpretation form and save PDF file with patient first and last name.
2. For digital images: Email completed PDF request to: ristmvi@yahoo.com
3. For hard copy X-rays: Print the completed request and send with rads to MVI.
4. Digital images can be sent by email, on CD, or digital upload. Please call for details.
Mobile Ultrasound requests:
1. Call 902-403-7237 to schedule an appointment.
2. Complete ultrasound request form, print and have ready during the ultrasound exam.
3. If you want fine needle aspirates or biopsies on your patient, please download and have owner sign the owner consent form above.
